Rapid Care Plus Medical Membership Program Agreement

1. Membership Benefits:
The Rapid Care Plus Medical Membership Program provides access to urgent care medical services at
our designated Rapid Care facilities and through telehealth visits conducted online at RapidCareNY.com.
Membership benefits include: unlimited monthly visits, no copays, priority scheduling when appointments
are required, and full access to all of our urgent care medical services.

2. Membership Fee:
Enrollment in the Rapid Care Plus Medical Membership Program requires payment of a monthly
membership fee of $60.00 per enrolled person, as specified in the membership agreement.
The membership fee is non-refundable and must be paid in full to access program benefits.

3. Scope of Covered Services:
The membership program covers urgent care medical services exclusively and does not replace
comprehensive health insurance coverage.
Additional medical services, prescriptions, or durable medical equipment not explicitly covered under the
membership program are not included and will be billed separately.

4. Usage Limitation:
The Rapid Care Plus Medical Membership Program may only be used for urgent care services rendered
at the designated Rapid Care facility or through telehealth visits conducted online via our secure platform.
Services obtained outside of these authorized locations are not covered under the membership program.

5. Non-Insurance Use:
The membership program cannot be used to fulfill or contribute towards any health insurance deductible,
copayment, or coinsurance requirements.
Members are responsible for maintaining active health insurance coverage to cover medical expenses
beyond the scope of this membership program.

6. Privacy and HIPAA Compliance:
We are committed to protecting the privacy and confidentiality of our members' personal and medical
information in accordance with applicable privacy laws, including HIPAA.
All patient information is kept confidential and protected, and access to patient records is limited to
authorized personnel for the purpose of providing medical care and administrative functions.

7. Payment Processing and Responsibility:
Members are required to maintain a valid and functioning credit or debit card on file for payment
processing purposes.
The patient authorizes the initial application and automatic recurring billing for membership fees and any
applicable charges for non-covered services or late payments.

The patient is responsible for ensuring that their payment information is accurate and up-to-date at all
times. In the event of late or missing payments, the patient agrees to be responsible for any costs
incurred by the provider for collections, including but not limited to collection agency fees, legal fees, and
court costs.

8. Membership Program Changes and Updates:
We reserve the right to modify, update, or terminate the Urgent Care Medical Membership Program at
any time, with or without prior notice to members.
Changes to the program terms and conditions will be communicated to members through written
notification or updates to our website.

9. Membership Renewal and Termination:
Membership in the Rapid Care Plus Medical Membership Program is automatically renewed and billed
monthly. Members may cancel their membership at any time by providing written notice to our administrative office
addressed to: Rapid Care, 815 NY-208, Monroe, NY 10950.

Refunds for prepaid membership fees will not be provided for cancellations.

By enrolling in the Rapid Care Plus Medical Membership Program, you acknowledge that you have read,
understood, and agree to abide by the terms and conditions outlined in this membership agreement. If
